Sheetal’s Skin Care requires new clients to fill out a Client Consent Form, located at the bottom of the home page of this site, and on this page as well. Please download the form to your scheduled treatment and bring the filled out form to your appointment. This consent must be signed before your appointment begins. Please bring with you all skin care products you are currently using on your skin. Please inform your Esthetician of all your medications you are taking or have taken in last 3 months. Also if you are using any prescriptions like Accutane, Retina-A, Renova, Waxing done in last 72 hrs
Kindly allow 24 hour notice in the event it is necessary to cancel an appointment to avoid a cancellation fee.
Cancellation fee – A full Treatment price will be charged as a cancellation fee.
